Community Health Worker (CHW) – Multiple Full Time positions available with Health Access Project (HAP). Duties include interacting with community members living in Salt Lake County affected by COVID-19 and connecting them to needed resources. The Community Health Worker will act as a liaison between the County Health Department to contact patients for the purpose of contact tracing and education related to COVID-19.
This position requires one of the following: (1) A Bachelor’s degree in social work, psychology, public health, or a related field and at least one year of experience providing direct community health worker services to a comparable client base. (2) A minimum of 90 semester units with an educational major in social work, psychology, public health, or a related field and at least three years of direct community health worker experience.
Bilingual skills in English and the language(s) of the community being served with advanced computer literacy in Microsoft Office (Word, Excel, etc.) are required. Must be well organized, able to multitask, and have excellent written and verbal skills. Experience working in a health care setting with diverse populations is preferred.
